Please ensure that you fill out the intake form prior to your appointment as there are important questions that could rule you out as a treatment candidate. If the form is not filled out prior and you arrive at your appointment and we discover a contraindication (meaning you can’t receive treatment) this will be considered a late cancellation as it is entirely preventable and therefore you will be charged. PLEASE fill out the form and message me if you notice anything that would rule you out (travel, antibiotic use, dentist appts, recent vaccines, etc)
